General Requirements

• An average grade of at least 85% or its equivalent, with at least an 80% grade in all subjects
• Income tax return not exceeding P300,000, combined gross annual income of parents
• Photocopy of birth certificate with the original copy (for verification)
• Photocopy of utility bills with the original copy (for verification)
• Endorsement letter from the school
• Two 2×2 and 1×1 ID pictures
• Photograph of the applicant with family or guardian
