Eligibility Requirements
To qualify for the Dr. Pio Valenzuela Scholarship Program, applicants must meet the following academic standards:
Academic Performance
Applicants must have:
- No grade lower than 85 in any subject
- General Weighted Average (GWA) of 85% and above
From the following:
- Grade 12 Report Card (1st Semester)
- Grade 11 Report Card (1st & 2nd Semesters)
Financial Eligibility
The scholarship prioritizes students from low-income households.
If Parents Are Employed
- 2024 Income Tax Return (ITR 2316)
- Annual Gross Income must not exceed ₱120,000
- Latest 1-month payslip
If Parents Are Unemployed
- Affidavit of Non-Filing of ITR
- Must be JOINT if both parents live together and are legal guardians
- Purpose must state: For the Dr. Pio Valenzuela Scholarship Program
- Certificate of Non-Filing of ITR
If Parents Are Working Abroad (OFW)
- Employment Contract
- Latest 2 months’ remittance records
Other Required Documents
Applicants must also prepare the following:
- Certificate of Residency and Indigency of both parents (from Barangay)
- Picture of the actual street and residence of the applicant
- Hand-drawn residence sketch with landmarks from PLV Maysan Campus to home (Google Maps screenshots are NOT allowed)
- PSA Certified Birth Certificate
- Certificate of Good Moral Character (for DPVSP)
- School ID Card (front and back)
- Voter’s Certificate of registered parent/guardian (one only)
- Voter’s Certificate of applicant (if 18 years old or above)
