• Must have a general weighted average of 80% or higher.
• Must not have grades below 75% in any grading period (grades for MAPEH will be assessed individually).

• Payslip or ITR/Certificate of Tax Withheld of all working family members (if available).
• Certificate of Residency from the barangay, stating that the family has been a resident for at least 1 year.
• Pantawid ID, if the student is a DSWD Pantawid beneficiary.
• Autobiography:
  • Minimum of 1,500 words, typewritten, using Times New Roman font size 12.
  • Must include personal life experiences, turning points, crises, beliefs, ideologies, future plans, talents, assets, strengths, and weaknesses.
  • Note: Autobiographies with less than 1,500 words will not be accepted.
• Sketch of home address:
  • Start from a major landmark going to your home address.
  • Indicate barangay and street names for easy identification (Google Maps will not be accepted).
