To apply for the NU-Siblings Discount Program, students must submit the following requirements:

• Accomplished Scholarship Form from the Student Development and Activities Office
• Photocopy of the Certificate of Registration (COR) of the siblings
• Photocopy of PSA-issued Birth Certificate for each sibling

• Complete Report Card (1st to 4th Quarter) — must be scanned front and back clearly in PDF format
• For AMLIG applicants — submit a recommendation letter instead
• Applicants with incomplete grades will be automatically disqualified (except AMLIG)
