Requirements:

To qualify for the Provincial Government of Quezon Scholarship, applicants must submit the following documents.

• Letter of Intent – A formal letter expressing the applicant’s intent to apply for the scholarship.
• Duly accomplished Application Form – The official scholarship application form, properly filled out.
• Proof of Identity (submit any of the following):
  • PSA Birth Certificate – A certified copy of the applicant’s birth certificate issued by the Philippine Statistics Authority.
  • Photocopy of PhilSys ID – A copy of the applicant’s National ID. (The original ID must be presented during registration for verification.)
• Academic Records:
  • Certified Photocopy of DepEd School Form 9 or Learner’s Progress Report Card – The applicant’s latest academic record, signed and sealed by the School Principal.

Step 6: Final Selection
