Requirements:

To apply for the SM Employee and Affiliate Discount, applicants must specifically submit the following requirements:

For Employees of SM and its Affiliates:

• Duly signed Certificate of Employment (COE) from the HR Department of SM or its affiliates
• Photocopy of the employee’s company ID
• PSA-Authenticated Birth Certificate of both the employee and the student applicant/enrollee

For SM Foundation Scholarship:

• Endorsement letter from SM Foundation

Student Enrollment Requirements:

• Complete Grade 12 Report Card (Form 138)
• Transfer Credentials or Transcript of Records (TOR) for Transferees, 2nd Degree Takers, and Graduate Studies
• Certificate of Good Moral Character
• PSA-Authenticated Birth Certificate
• Recent 2×2 Colored ID Picture with white background

• Complete Report Card (1st to 4th Quarter) — must be scanned front and back clearly in PDF format
• For AMLIG applicants — submit a recommendation letter instead
• Applicants with incomplete grades will be automatically disqualified (except AMLIG)

• White Scholarship:

  • May apply for other scholarships by 2nd year, if eligible and slots are available
• Other Scholarships:
  • Renewable every term until graduation
  • Must meet grade requirements
