- A bona fide Filipino citizen and a resident of General Santos City
- A Senior High School graduate from any recognized school in General Santos City
- Graduated with Highest Honors during Senior High School
- Currently enrolled in any recognized tertiary school within General Santos City
- Combined annual gross income of parents/guardians for CY 2023 must not exceed ₱250,000
- If income exceeds ₱250,000, applicant must provide:
- A written certification or medical findings of a family member’s illness, or
- School certifications showing two or more dependents enrolled in college

- PSA-authenticated Birth Certificate (original and photocopy)
- One (1) 2×2 colored photo with white background
- Certificate of Residency issued by the barangay
- Photo of residence (full view showing the house and house number)
- Proof of billing from the last two (2) months
- Grade 12 Report Card (photocopy)
- Certification from school confirming the applicant graduated with Highest Honors
- Certificate of Good Moral Character (photocopy)
- Latest Income Tax Return (BIR Form No. 2316 for CY 2023) of parent/guardian
- If not available, submit a Certificate of Tax Exemption from BIR or a Certificate of Indigency from the barangay, indicating the parent/guardian’s annual income
- Endorsement letters from two (2) disinterested persons, attesting to the applicant’s character and financial circumstances.
